How to Refer a Patient

Send the Referral

A brief note naming the clinical concern, age, urgency level, and any imaging the patient already has. Email, fax, or phone. The Limestone Hills front desk handles scheduling and insurance verification from there.

Free Consultation

The referred patient receives a 3D CBCT scan, an intraoral (iTero) scan, clinical photographs, and a treatment plan with options, timeline, and fee in writing. No consultation fee. No same-day commitment required.

Written Report Back

The referring office receives a written report at the consultation, at start of treatment, and at every milestone (placement of appliances, mid-course adjustments, finishing, retention handoff). The patient is not the messenger.

Comprehensive Phase 1 (ages 7 to 10): expansion, facemask, functional appliances
